Patient Rights & Responsibilities

A. Patients are treated with respect, consideration and dignity.

B. Patients are free from all forms of abuse, neglect and harassment.

C. Patients are provided appropriate privacy.

D. Patients are entitled to care in a safe setting.

E. Patient disclosure and records are secured at all times and treated confidentially, except when required by law, patients are given the opportunity to approve or refuse their release.

F. Patients are provided, to the degree known, complete information concerning their diagnosis, evaluation, treatment & prognosis. When it is medically inadvisable to give such information to a patient, the information is provided to a person designated by the patient or a legally authorized person.

G. Patients are given the opportunity to participate in decisions involving their healthcare, except when such participation is contraindicated for medical reasons.

H. Patients have the right to change providers at their request.

I. Patients are fully informed about a treatment or procedure and the expected outcome before it is performed.

J. In the event that a physician(s) does not have medical malpractice coverage, patients will be notified.

K. Marketing and Advertising regarding the competence and capabilities of the organization will not be misleading to patients.

L. Patients are informed about procedures for expressing suggestions, complaints and grievances.

M. Patients may exercise their rights without being subjected to discrimination or reprisal.

N. Information is available to patients and staff concerning:

1. Patient rights including those specified in A, B, C, D and E above.

2. Patient conduct and responsibilities.

3. Services available at the organization.

4. Provisions for after hour and emergency care.

5. Fees for service.

6. Payment policies

7. Patient’s right to refuse participation in experimental research.

8. Advance directives, as required by state or federal law and regulations.

9. The credentials of health care professionals.

Patient Responsibilities

A. Provide complete and accurate information to the best of his/her ability about his/her health, any medications including over the counter products and dietary supplements and any allergies or sensitivities.

B. Follow the treatment plan prescribed by his/her physician.

C. Provide a responsible adult to transport him/her home from the facility and remain with him/her for 24 hours, if required by his/her provider.

D. Inform his/her provider about any living wills, medical power of attorney, or other directives that could affect his/her care.

E. Accept personal financial responsibility for charges not covered by his/her insurance.

F. Be respectful of all health care providers and staff, as well as other patients.
